Volleyball Academy Application Form

This digital application form is intended to gather basic contact information, goals, pre-order athletic apparel. You will be notified by e-mail upon a successful or unsuccessful application. You will be contacted via email by May to see if you have been successfully accepted or not.

Please fill out this form for each student applicant.

TUITION:
*The 2025-26 tuition is $650 ($112.50/month, over 4 months Feb - May)
*A $200 deposit is required for each participant

*Opt to pay full tuition by May 31st to receive a LPSD Academy Hoodie
