Sports Academy Application Form

This digital application form is intended to gather basic contact information, goals, pre-order athletic apparel. You will be notified by e-mail upon a successful or unsuccessful application. You will be contacted via email if you have or have not been successfully accepted by May.

TUITION:
*The 2025-26 tuition is $700 ($62.50/month, over 8 months Sept - April)
*A $200 deposit is required for each participant

*Opt to pay the full tuition by May 31st and receive a LPSD Academy Hoodie
